COVID-19 info

Due to the current situation with coronavirus and to protect all patients and staff we are now doing all appointments by telephone triage. Please DO NOT attend the surgery, the clinician will ring you. Your cooperation is appreciated. Appointments for dressings, some type of injections can still be booked but all patients will be telephoned beforehand. The open access clinic will be closed until further notice.  Due to the current situation and to prevent a shortage  we will only be issuing 28 days supply of ALL medications.    Following guidance from NHS England the BMA and RCGP our Doctors will not be carrying out the following during the outbreak of COVID-19  INSURANCE REPORTS AND CLAIM FORMS, NON-URGENT PAPER WORK AND DVLA MEDICAL EXAMINATIONS. This service will resume from the 1st April 2021

During the coronavirus outbreak, we will only be accepting registrations by email or post. Please download a copy of the registration form from our website Elizabethcourtauld.gpsurgery.net. Please then scan the registration form and send together with one form of photo ID and a recent utility bill showing your current address to f81068.reception@nhs.net  Alternatively, please post your completed form and a photocopy of your photo ID and recent utility bill to:

Do not enter the surgery if you have:

  • A high temperature
  • And/or a new continuous cough

Please go home and self-isolate for 7 days

Face coverings update (19/07/21)

Following the Government’s announcement today to say rules will be relaxed from 19/7/21, please note that we will still require you to wear a face mask when attending the surgery unless you are exempt. This is to protect our staff and patients. The number of COVID cases are rising, not decreasing and we cannot risk our staff contracting Covid and potentially having to close the surgery. Thank you for abiding by our practice rules.

COVID-19 vaccination status

From 17 May 2021, people in England who have been fully vaccinated against COVID-19 can demonstrate their vaccination status for international travel. A full course is currently two doses of any approved vaccine. Vaccine status will be available from:

  • the NHS App which you can download from app stores
  • the NHS website (from 17 May 2021)If you cannot access these online services, and you have had 2 vaccines, you can request a paper letter from the NHS by calling 119. Only call 119 if you had your second dose more than 5 working days ago. It may take 7 working days for the letter to arrive.
  • This practice is not able to provide you with a letter that shows your COVID-19 vaccination status.
  • It may take more than a week for your identity to be checked and verified so you can use these services.
