How to register

In order to register with the practice, you should bring your NHS Medical card to the surgery. If you do not have a medical card a form can be provided. If you are on medication, you can request a phone call from your doctor who will dicuss your needs and arrange a new patient appointment if appropriate.

Two forms must be completed before you can register with our practice.

Form 1 – The ‘New Patient Questionnaire’ form can be downloaded by clicking here: New Patient Questionnaire 07-2020  for you to complete before your visit to the surgery.

Form 2 – The GMS1 form is required by the Health Authority , please click on the link to download and complete before your visit to the surgery.

Form 3 – From 30th April 2019 GP practices will no longer be able to input Patient Donor Preferences, if you would like to become a donor please click here for more information.

You will not be registered at this practice until both forms have been completed.

When you register with us, if you give us your mobile phone number, please be aware that we may use it to send you SMS messages to remind you about appointments, let you know if you have missed an appointment or send you test results. if you do not wish us to do this, please tick the appropriate box on the new patient questionnaire which you will be given before you register. For existing patients who do not wish to recieve SMS messages from us, please contact the surgery and let us know that you wish to opt out from receiving SMS messages.
